Propafenone

Abstract
Propafenone hydrochloride (propafenone), a class IC antiarrhythmic drug, has been widely used for the treatment of ventricular arrhythmias in the 1980s and early 1990s, and for supraventricular arrhythmias in the late 1990s in Europe and the United States. Propafenone is rapidly absorbed from the gut when taken orally and is then metabolized by the liver. This drug is a sodium-channel blocker whose kinetics of interaction with sodium channels are similar to those of other class IC antiarrhythmic drugs, like flecainide and encainide. Unlike others, it acts as a ¥â-adrenergic antagonist. The drug is administered as a racemic mixture, and the 2 enantiomers have different pharmacological actions and kinetics of disposition. Propafenone, like other antiarrhythmic drugs, has cardiac and noncardiac adverse effects. In some cases, propafenone could worsen sustained ventricular arrhythmia and cause further lethal arrhythmias.
